Team Check-In will be held at the Embassy Suites on International Blvd from 6pm - 9 pm.  Late arriving teams or teams driving up on Saturday morning may check-in at the Main Site (JIYSC) no later than 1 hour before their first match.

You will need the following items for Team Check-In:

1. Original approved state roster and 4 copies. You must have players jersey number on each of the rosters except your Original State roster.  All rosters will be checked at team check in and will be approved for play.  You must bring your approved copy to each game to give to the Referee.  You will not get these back.  Tournament will keep for recording cautions and cards to your state. TEAMS WITH GUEST PLAYERS MUST USE THEIR STATE’S TOURNAMENT ROSTER FORM + 4 COPIES.

2. Laminated/validated 2009/2010 player passes.

3. Medical Release/Conduct Form  - Teams may use Medical Release Forms from their State Associations - Teams that do not have a form from their State Association should use the tournament’s form (download below)

4. Permission to Travel/Intent to Travel Form from State Association 

5. All required guest player information to include: Laminated/validated 2009/2010 player pass for each guest player, medical release form & guest player form (Guest players MUST meet all requirements that all other teams and players are required to meet).
